Of course there are a lot of strange guys in big cities, Moscow city has also some. This one was submitted by Levych. But maybe he needed some medical help, and those guys on the background had to call the ambulance instead of laughing on him… See the video below:

    Looks like a retired proletarian who just converted his pension into vodka. Nothing strange. People like this are to be found on every corner in Russian cities, in particular in the countryside area.

    He does not look like drunk to me. He has no problems in balance, and his gestures are quite accurate, like adjusting his jacket and taking off his hat.

    These symptoms might be related with alcohol, but there is definitely something else too explaining this.

    My bet is he has some sort of either mental and/or physical disorder that produces unvoluntary movements.

    Basically he seems otherwise OK, ie. no need for an ambulance or anything. Not an emergency anyway.
